Student Medication 

A signed consent form must be completed and turned into the clinic along with the medication. You can view and download the form by clicking here.  

For medication to be administered at school, students must have:

  • Written permission with the date, student’s name, the name of the medication and reason it is being given, and the parent/guardian’s signature.
  • A note including the time of day the medication is to be administered and the number of days the student is to receive the medication.
  • Prescription medication must be in its original prescription bottle with the Doctor’s name and instructions clearly written on them by the pharmacist. 

Over-the-counter medication must be in the original bottle with age appropriate dosage guidelines on the container.

Note: The pharmacist will give you an extra bottle when you get the prescription filled if you tell them your child will be taking this medication at school.

  • No loose pills or unlabeled medicines will be given to students under any circumstances.

·        No expired medication will be administered under any circumstances.

Illness 

Parents will be notified and students will be sent home if the student has a temperature of 100’ F or above and/or is vomiting. The student must be free of fever for 24 hours before returning to school. 

Please make sure that your school nurse is aware of any health condition that your child may have, such as diabetes, asthma, seizure disorders, allergies or any other mental or physical health concerns.  

Emergency Card 

Please remember to call or send a note updating the emergency card as information changes. That would include work or home phone numbers and names and numbers of people you have given permission to pick up your child.  

Head Lice 

Anderson-Shiro CISD proposes to enforce a no nit policy in order to control the spread of lice in our schools and maintain a safe and healthy environment for our students. Those students found to have nits/lice will be sent home from school. The student must be treated at home and will be allowed back in school only after they have been examined by the school nurse and found to be free of nits and lice. Parents will have only one day excused absence per occurrence to treat student’s hair for head lice/nits. All lice and nits must be removed.

Immunizations 

  • All immunizations should be completed by the first day of attendance at school.
  • The law requires that students be fully vaccinated against the specified diseases.
  • It is a state law that your child's immunization record be kept up-to-date and on file in the school office.
  • A student can be enrolled provisionally for no more than 30 days if he/she is transferring from another Texas school.
  • Student’s transferring from an out-of state school must have their completed immunization record in the school office before they can be admitted to the school.

  • Meningitis vaccine is recommended for all children at their preadolescent visit with their doctor. If your child has never received the vaccine, it is recommended before they start high school.
