Due to Senate Bill 12, all parents/guardians must complete new Nurse and Counselor Consent Forms for the 2026-2027 school year.
Please note: The consent forms completed last school year cannot be carried over or reused. A new form is required for every student each school year.
We appreciate you taking a few minutes to complete them as soon as possible so our nurses and counselors can continue providing the support your child may need throughout the school year.
